Program Eligibility
Our support programs are designed to serve individuals with various needs. Eligibility requirements may vary by program.
To be eligible for the P/FDS Waiver Program, individuals must:
- Be diagnosed with an intellectual disability
- Meet specific functional eligibility criteria
- Be a resident of Pennsylvania
- Meet financial eligibility requirements for Medical Assistance
- Be 3 years of age or older
Specific eligibility determination is conducted through your county's Office of Intellectual Disabilities.
Individuals interested in the Life Sharing Program should:
- Have an intellectual disability diagnosis
- Be eligible for waiver funding or have alternative funding sources
- Be able to benefit from a family living environment
- Require support that can be provided in a shared living arrangement

Behavioral Support services are available to:
- Individuals enrolled in other Care in Minds programs
- Those with identified behavioral support needs
