Program Eligibility

Our support programs are designed to serve individuals with various needs. Eligibility requirements may vary by program.

To be eligible for the P/FDS Waiver Program, individuals must:

  • Be diagnosed with an intellectual disability
  • Meet specific functional eligibility criteria
  • Be a resident of Pennsylvania
  • Meet financial eligibility requirements for Medical Assistance
  • Be 3 years of age or older

Specific eligibility determination is conducted through your county's Office of Intellectual Disabilities.

Individuals interested in the Life Sharing Program should:

  • Have an intellectual disability diagnosis
  • Be eligible for waiver funding or have alternative funding sources
  • Be able to benefit from a family living environment
  • Require support that can be provided in a shared living arrangement

Our team works closely with each individual to determine if Life Sharing is an appropriate option based on their unique needs and preferences.

Behavioral Support services are available to:

  • Individuals enrolled in other Care in Minds programs
  • Those with identified behavioral support needs
  • Individuals with intellectual disabilities or autism
  • Those with appropriate referrals and funding

A comprehensive assessment will be conducted to determine the appropriate level and type of behavioral support needed.
